The Most Common Signs of Low Testosterone in a Man

Low testosterone doesn’t usually show up as one dramatic symptom. It’s more often several smaller things that build up over time. The most common signs of low testosterone in a man include:

  • Difficulty sleeping at night

  • Fatigue during the day, especially a midday energy crash

  • Reduced mental focus or brain fog

  • Anxiety or depression

  • Weight gain, especially around the midsection

  • Difficulty losing weight no matter what you try

  • Decreased muscle strength or trouble building muscle

  • Muscle and joint pain

  • Reduced sexual desire and performance

Sound familiar? If you notice more than one of these symptoms—even if each one individually seems minor or explainable on its own—it’s worth getting a blood test to check for low testosterone.

Why Hormones Are Always the Last Guess

Almost every symptom on the list above can be explained away. Tired all the time? Must be bad sleep or too much stress. Gaining weight around the middle? Must be your diet or the fact that you’re getting older. Losing muscle despite hitting the gym regularly? Must just be time to train harder.

For many patients we see, the signs and symptoms of low testosterone in men are frequently dismissed this way for years, since any one of them could have a dozen other explanations. What tends to be the real signal is the combination—fatigue, mood changes, stalled muscle gains, and low libido showing up together, rather than any single symptom in isolation.

It’s worth considering hormone testing sooner rather than later if you have a combination of the above signs and symptoms.

Low Testosterone Symptoms in Men vs. Normal Aging

It’s true that energy, muscle mass, and libido naturally shift with age, but low testosterone symptoms in men aren’t just “getting older.”

Normal aging tends to be gradual and mild. But whether your symptoms crept in slowly over the years or showed up more suddenly, both are worth a blood test to check your testosterone levels. Gradual doesn’t mean you’re just aging, and sudden doesn’t mean it’s automatically something else.

The only way to actually tell the difference is bloodwork—both total and free testosterone levels—rather than guessing based on symptoms alone or assuming it’s just part of getting older.

Why “Normal” on a Lab Report Isn’t Always the Full Story

A testosterone test showing a “normal” number doesn’t always mean much on its own. Total testosterone counts everything in your bloodstream, including testosterone that’s bound to a protein called sex hormone-binding globulin (SHBG)—and bound testosterone is essentially inactive. It doesn’t do anything for your energy, libido, or muscle mass. What actually matters is your free testosterone, the portion that’s unbound and available for your body to use.

Two men can have the exact same total testosterone number and feel completely different, simply because one of them has higher SHBG tying up more of it. If a provider only checks total testosterone, they can hand you a “normal” result while you’re still dealing with every symptom on the list above.

In addition, reference ranges on a standard lab report are wide, sometimes spanning from around 300 to 1,000 ng/dL as “normal.” Technically clearing that bar doesn’t mean you’re at the level where you actually feel good. A number sitting at the low end of “normal” can still leave you fatigued, foggy, and with a low sex drive, which is exactly why the specific number matters more than whether it falls somewhere inside a broad range.
